Patient Payment: First Six Appointments + DNA Test

Your first appointment in this package is a 90 Minute Intake Appointment
Your second through sixth appointments in this package are 60 Minute Follow Up Appointments
Here is what each appointment will look like for the patient:
The 90 Minute Intake Appointment
At the Intake Appointment Dr Jeni works like a detective to gather specific information from the patient to understand the patients WHOLE (or, holistic) health picture to ultimately determine the Root Cause of the patients specific symptoms. This appointment is the most important appointment that a patient will experience. An Intake appointment is not designed to GIVE health advice, but rather to GATHER information to design the custom protocol that the patient needs to properly address the Root Cause of their symptoms and health concerns and to help them to reach their health goals. Prior to this appointment, Dr Jeni prefers to have 7-10 days of Wellness Journals filled out. Patient Intake forms will be sent electronically for authorization.
The First 60 Minute Follow Up Appointment
At the first follow up Appointment, typically within 7 days of the Intake Appointment Dr Jeni will present the custom protocol to the patient. Each protocol is designed to directly addresses the Root Cause of the patients symptoms. The custom protocol that Dr Jeni presents to the patient is based on the information received at the Intake Appointment.
Four (4) additional 60 Minute Follow Up Appointments
These appointments are used to offer regular updates to your protocol. It is recommended that the patient schedule these appointments every 7-14 days. Once the protocol has been presented to the patient, often adjustments need to be made and advice is required to check in on the progress of the protocol. These appointments are scheduled to last up to 60 minutes each. If the appointments last longer than 60 minutes, there is no additional charge to the patient.
All major credit/debit cards are accepted including Health Savings Accounts (HSA), PayPal, ACH Transfer, or Cash/Check.
Once payment has been made, if a refund is desired, a $125 Administration Fee will apply. There are no refunds available on any testing that is ordered. The full cost of testing and wellness reports plus a $125 Administration Fee will apply if a refund is desired.
